This role will work directly with patients, offering personalized nutrition and wellness counseling to reduce symptoms and improve quality of life.This role will empower patients with the knowledge and tools to make lasting behavioral changes that will improve clinical outcomes and overall wellness. You'll serve as one of our patients' primary success partners, with responsibilities including: addressing member health questions, providing relevant educational resources, and fostering positive member accountability. What You Will DoProvide exceptional nutrition counseling, disease management and health education to patientsSupport goal setting for individual patients both synchronously and asynchronously to help them better manage their chronic conditions and associated dietary needsCreate personalized action plans with guidelines to reduce or eliminate unwanted behaviorsHelp clients understand their motivations and create behavior change plans Conduct regular outreach to patients, based on their needs and preferences, to support engagement and elicit behavior changeCoordinate with clinical team to provide an exceptional patient experienceDevelop and maintain professional, support-oriented working relationships with patients and team membersCreate and revise consumer-facing content advising on nutrition best practices for chronic GI conditions and weight managementTranslate complex medical ideas into digestible messages for a lay audience, including performing research as required to ensure understandingInterpret clinical protocols and recommendations into relatable, concise educational content for Salvo's appFollow the nutrition care process, and complete documentation in a timely mannerCreate and distribute health education materials to individual members when necessaryRequirementsRD/RDN, Multiple state licensure a plus (FL, LA, OH, IL)2+ years experience as a an RD/RDNBachelor's degree required, Master's Degree PreferredExperience communicating with patients asynchronously preferredExperience with GI conditions and weight management a plusPassion for working with clients to facilitate behavior change by helping them set health goals and modify food intake to effectively manage chronic disease.Experience working with patients in a clinical setting preferredDemonstrates a willingness to go above and beyond normal responsibilities to provide the best care possibleExcellent interpersonal communication and problem-solving skillsExceptional organizational skillsExperience and ability to thrive in a fast-paced, high-growth environmentStrong personal computing skills - remote work experience preferredPassion for and commitment to functional medicine approachesSalvo is a new approach to help millions of Americans facing chronic health conditions, centered on chronic gut health and metabolic conditions from IBS to obesity.
