Job Duties:
Coordinate nutritional care of residents and patients by completing nutritional assessments.
Participate in care planning meetings as required.
Provide nutritional counseling and education for residents and family.
Completes comprehensive assessments on patients as required.
Provides dietary in-service training as required.
Under PDPM, identify, document, and treat nutrition related diseases for improved outcomes.
Meet DOH/state/federal and company standards.
Perform duties independently without training or direction.
Perform other duties as assigned by management.
Required Skills/Abilities:
Good decision making and judgment capabilities.
Ability to identify and solve complex problems.
Great communication and interpersonal skills are a must.
High comfort level with enteral nutrition, wounds, weight management, Hemodialysis.
Comfortable working in a healthcare facility.
Familiarity with state survey process and compliance with nutrition care regulations.
Education and Experience:
Bachelors degree in nutrition, health, or related field.
Must possess Registered Dietitian licensure within the state of operations
At least one year of experience in a long-term care setting
Experience with MDS 3.0.
Experience with electronic charting such as Point Click Care is a plus
Physical Requirements:
Prolonged periods standing and walking.
Must be able to lift up to 25 pounds at times.
