Job Description: Responsible for nutritional care of patients. Plans, assesses, implements, and evaluates patient nutritional care plan. Assists in menu modifications and other nutritional related functions. Pioneering Role Development: Take initiative in shaping this newly position in a unique setting and new inpatient unit. Food Service Liaison & Process Improvement: Serve as the primary liaison between the clinical nutrition team and the food service department, fostering collaborative relationships to ensure seamless meal service operations. Proactively identify and implement process improvements to enhance efficiency, minimize food waste, and maximize patient satisfaction. Addressing Food Insecurity: Develop and implement strategies to address food insecurity among patients, connecting them with appropriate resources and support services both within the healthcare system and in the community. Collaborate with social workers and case managers to ensure a comprehensive approach to patient care. Therapeutic Diet Expertise and Patient Education: strong emphasis on accommodating therapeutic diets and addressing food allergies/intolerances. Provide comprehensive nutrition education and counseling to patients and their families. CBORD Proficiency (Preferred) : Experience with CBORD food service management software is highly preferred, allowing for efficient management of patient meal orders, nutritional analysis, and inventory control. Willingness to learn and become proficient in CBORD is essential. Quality Improvement & Data Analysis : Actively participate in quality improvement initiatives, collecting and analyzing data. Embrace opportunities to contribute to the growth and development of the Nutrition Services department. Job Responsibility 1.May mentor and train team members.2. Responsible for accurately and responsively performing varied clinical tasks and problems of minimal complexity3.Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients' medical record.4. Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration.5.Recommends therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician's order.6. Provides nutritional guidance, education and counseling to patients and their families in accordance with the prescribed diet. Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians.7.Contributes to changes to patient's menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handing and portion control.8. Supports a clinical function to assist clinical team members and department or unit management with carrying out daily clinical operations9. Prepares, presents, and conducts individual and group education classes, and refers clients to community services, as needed. Participates in and promotes Northwell wellness initiatives.10.Develops and utilizes effective lines of communication regarding nutrition information within the program and community.11.Provides in-services to culinary, food service and interdisciplinary teams regarding therapeutic diets and related topics.12.Participates in Quality Assurance/Performance Improvement related tasks including data collection and executes tasks related to it, as directed.13.Solves or escalates operational problems and technical issues to senior team members and management; modifies existing tools or processes14.Performs related duties as required.
