Our goal is to change the approach to pre-condition and chronic condition care management, to use technology to support health-care providers in improving patients' health outcomes. We have an opening for a part-time Registered Dietitian Nutritionist (RDN) on the Brook Health Telenutrition coach team. The telenutrition coach team focuses on providing users with access to nutrition professionals via video-based appointments. Brook's telenutrition service augments the other services we offer by keeping users engaged and on the platform. These services include Remote Patient Monitoring (RPM) in clinics and hospitals, a digital Diabetes Prevention Program (DPP), and our Brook Health Companion app. Whether they are dealing with a chronic disease, such as diabetes or hypertension, or are just looking to make healthier choices, the telenutrition service enables our users to receive targeted support and form a personal connection with their RDN coach. This position is part-time at 29 hours per week, work from home, remote work.This position requires you to already have a license in dietetics-nutrition from the State of New York. - Be customer-centric in your approach

- Licensed as a dietitian-nutritionist in New York state
- Ability to be credentialed by insurance
- Experience with outpatient counseling for general wellness, diabetes, hypertension, and diabetes prevention
- Experience with GLP-1 or other weight loss medications
- Clean OIG exclusions search
Preferred:
- Licensed as a dietitian-nutritionist in Massachusetts state
- Experience delivering outpatient counseling via digital messaging and remote video services
- Proficiency in writing and editing educational materials
Once onboard, this person will be trained to utilize our telenutrition software and app. Brook will assist in the process of credentialing this person with our health insurance customers.
