Job Description Responsible for nutritional care of patients. Plans, assesses, implements, and evaluates patient nutritional care plan. Assists in menu modifications and other nutritional related functions. Per Diem - Needed Monday and Friday from 9am-5pm. Additional days may vary. Job Responsibility
  • May mentor and train team members.
  • Responsible for accurately and responsively performing varied clinical tasks and problems of minimal complexity
  • Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients' medical record.
  • Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration.
  • Recommends therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician's order.
  • Provides nutritional guidance, education and counseling to patients and their families in accordance with the prescribed diet. Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians.
  • Contributes to changes to patient's menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handing and portion control.
  • Supports a clinical function to assist clinical team members and department or unit management with carrying out daily clinical operations
  • Prepares, presents, and conducts individual and group education classes, and refers clients to community services, as needed. Participates in and promotes Northwell wellness initiatives.
  • Develops and utilizes effective lines of communication regarding nutrition information within the program and community.
  • Provides in-services to culinary, food service and interdisciplinary teams regarding therapeutic diets and related topics.
  • Participates in Quality Assurance/Performance Improvement related tasks including data collection and executes tasks related to it, as directed.
  • Solves or escalates operational problems and technical issues to senior team members and management; modifies existing tools or processes

  • Job Qualification
    • Master's Degree or equivalent combination of education and related experience, required.
    • Current Registered Dietitian Certification with the Commissions on Dietetic Registration, required.
    • Certified Dietitian Nutritionist in New York State (CDN), as needed.
    • Advanced/specialized certifications, as needed.
    • 1-3 years of relevant experience, required.
    • Acute care experience, preferred.
