Registered Dietitian-Per Diem-Eddy Visiting Nurses
Eddy VNRA, a home health agency serving the Capital Region, is looking for a per diem Registered Dietitian to visit patients in need in their homes. This is a great chance to utilize your skills in the patients' own setting!

What you will do:
* Work with an interdisciplinary team to assist patients and their families in dietary needs for optimal health.
Responsibilities
* Work one on one with patients and families in their home setting or per televisit which allows you to customize their diets.
* Educate patients (and their families) on specialized diet and assist in weight management Y
* You may have the opportunity to work with MDs and outside agencies for pts with enteral feeding and TPN.
* Patients are referred following a hospitalization, inpatient rehab stay or by their physicians.
What you will need:
* Bachelor Degree in Nutrition/Dietetics (Didactic Program for Dietetics).
* Registered Dietitian Certification with New York State is preferred. ADA registration preferred but not required.
* Minimum of 1 year Dietitian experience preferably in an acute setting preferred
* Home care experience preferred but not required
