One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.Substitution:A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.Duties Description The candidate may clinically oversee a cluster of nurses in Brooklyn DDSO and be responsible for multiple sites and may function as a RN1 as needed.* Ensure nursing case management of multiple locations; caseload based on medical acuity and programmatic need- coverage when needed.* Oversee and supervise RN1s and LPNs who will supervise the AMAP staff in providing Direct Support Assistants health care per the Nurse Act Exemption.* Ensure all medical follow-up is done timely. Conduct random samples to ensure agency compliance.* Complete special projects as determined by Nursing Supervisor (i.e. AMAP tracking, Orientation, Pharmacy Review Tracking/Oversight, On Call follow up, Compliance Audits as directed, PONS compliance, Medication audits, Consent Tracking, EHR subject Matter Expert, etc.)* Engage in orientation and mentoring of new RN1s and LPNs as well as provide additional RN oversight and support in homes with noted deficiencies or incident follow up. Complete regular trainings as well as clinical trainings and assessments of RN1.Monitors individuals' medical/health needs and services.* Interacts with primary medical specialists, hospitals, and various clinical staff as necessary.* Advocate for individuals' health care needs.* Utilizes appropriate reporting mechanisms/chain of command in responding to individual and/or staff issues.* Responds to emergency requests as soon as possible (within and outside caseload).* Completes nursing assessments and related documentation via EHR, e.g., RN Notes, nursing logbook, etc., thoroughly and within established time frames.* Provides individual-specific nursing interventions.* Responds to staff questions regarding individual specific as well as regulatory and programmatic medical/nursing issues.* Provides health-related training/consultation to residential staff.* Participates in Nurse on call rotation.* Completes required medication error documentation and staff medication error remediation.* Participate in committees as designated by RN1 or DON.Maintains professional skills.* Attends Nursing Discipline meetings.* Attends/completes online mandated DDSO training, as well as available discipline-specific training.* Identifies problems and/or opportunities for personal and/or nursing department improvement (s) and contributes to developing an action plan.* Completes special projects/assignments.* Computer skills including Microsoft Word, Excel, and Electronic Health Record software.LinkedIn: Comments Please note this position covers all community residences (ICF and IRA).NY HELPS* This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.* If you are hired under the HELPS Program, you will be first be appointed as a temporary employee. In June 2024, if you are performing successfully in the position, your appointment will be changed from temporary to permanent non-competitive. Once you are in permanent status, your official probationary period will begin, and you may take any promotion exam for which you are qualified. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status.
