Minimum Qualifications Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Substitution:

A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.

Duties Description The candidate will clinically oversee a cluster of nurses in Brooklyn DDSO and be responsible for multiple sites and may function as a RN1 as needed.

* Ensure nursing case management of multiple locations; caseload based on medical acuity.

* Oversee and supervise RNs and LPNs who will supervise the AMAP staff in providing Direct Support Assistants health care per the nurse Act Exemption.

* Ensure all medical follow-up is done timely.

Responsible for in-service on nursing plans and other staff development and oversight as required, including medication administration, which are vital activities to regular operations.

* Engage in orientation and mentoring of new RNs and LPNs.

* Complete evaluations.

* Monitor/Complete nursing audit for the cluster, work closely with the Nursing Supervisor / Director of Nursing and Team to review findings, and develop a plan of correction.

Monitors individuals' medical/health needs and services.

* Interacts with primary medical specialists, hospitals, and various clinical staff as necessary.

* Advocate for individuals' health care needs.

* Maintains ongoing communication with med liaison/designee.

* Provides support to site staff in maintaining health services for the individuals.

* Utilizes appropriate reporting mechanisms/chain of command in responding to individual and/or staff issues.

* Responds to emergency requests as soon as possible (within and outside caseload).

* Completes nursing assessments and related documentation via elect, e.g., RN Notes, nursing logbook, etc., thoroughly and within established time frames.

* Provides individual-specific nursing interventions.

* Responds to staff questions regarding individual-specific medical/nursing issues.

* PONS is current (for caseload) and signed by the staff. All PONS are maintained in EHR.

* Provides health-related training/consultation to residential staff.

* Annually, for each person in your caseload, complete: a review of the medical section Life Plan/CFA, medication audit (each home/day program).

* Participates in Nurse on call rotation.

* Obtain individual's informed consent for medications, as per BDDSO Policy.

* Completes required medication error documentation and staff medication error remediation.

Maintains professional skills.

* Attends Nursing Discipline meetings.

* Attends/completes online mandated DDSO training, as well as available discipline-specific training.

* Identifies problems and/or opportunities for personal and/or nursing department improvement (s) and contributes to developing an action plan.

* Completes special projects/assignments.

* Computer skills including Microsoft word, excel and Electronic Health Record software.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.?

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

Posted Hiring Salary includes the sum of the statutory hiring rate ($72,032) for PS&T Grade 20, Geographic Pay Differential ($35,000), Increased Hiring Salary ($9,688) and Downstate Adjustment ($3,400). Maximum salary includes the sum of Job Rate ($91,381), Geographic Pay Differential ($35,000), and Downstate Adjustment ($3,400).

* If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

* The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

* Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.

* All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
