Our Community Health Nurses deliver care to patients in the community, providing a wide range of skilled nursing care while working to the top of their licensure. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, Occupational Therapists, Speech Language Pathologists, Social Workers and administrative support, all collaborating together to support patients to remain living in the setting of their choice, their home.Requirements - Greater than two years of home or public health nursing experience preferred, or a combination of education and experience from which comparable knowledge and skills are acquired.- Previous home care desired but not required. We are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.- Graduate nurses are considered for our Community Health Nurse I level.*Our coverage area serves Chittenden and Grand Isle Counties and is segmented into four territories. Exact case load territory is assigned after orientation period and may not be the same as listed on this posting*Requirements:
  • Current RN licensure recognized by the State of Vermont required.
  • Appropriate experience in specific clinical area. Varies by unit.
