Title: Registered Nurse - AmbulatoryLocation: Schwenksville PA 19473Duration: 26 weeksShift: (8:00 AM - 4:30 PM)IMPORTANT NOTE - Training will occur at 800 Walnut Street, Philadelphia PA. Once training is complete, practice location will be 102 Memorial Drive Schwenksville PA 19473. There is no set time for length of training, it is typically individualized based off the nurse's/practice needs. Summary:Strictly TELEPHONIC triage role: perform callbacks for sick/urgent concerns by working in the TriCounty Triage Nurse Pool. Responsibilities include:Triaging patients for acute/sick concerns via phone Providing patient educationScheduling for SDS visits or referring to urgent care/ED when appropriateCollaborating with providers and various members of the practice to maximize patient care outcomes Types of calls include URIs, rashes, GI related concerns, UTIs, vaginal bleeding, falls, orthopedic related concerns (knee pain, back pain) Looking for candidates with effective and strong communication skills, great with time-management, team player and able to work independently, empathetic, flexible, strong at navigating EHR while talking to patients on the phone On average, nurses perform anywhere from 40-60 callbacks per day.Average call length time is about 7 minutes.Details: The Ambulatory Nurse II Colleague is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model.The Ambulatory Nurse II Colleague possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Ambulatory Nurse II position description addendum. The Ambulatory Nurse II Colleague exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains.The Ambulatory Nurse II Colleague independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from infancy to the elderly. The Ambulatory Nurse II Colleague demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below).The Ambulatory Nurse II Colleague works closely with the interprofessional health care team to facilitate the coordination of care across the continuum.
