Summary: Under the supervision of the RN Supervisor, responsible for the supervision and delivery of health care services to consumers living in all certified sites based on providers' orders and good nursing practice. Essential Job Functions:
- Assists with the implementation, coordination, supervision and review of each consumer's nursing care plan, as directed by the primary care and other providers in compliance with OPWDD and The Arc of Livingston-Wyoming policies
- Respects consumer's rights to privacy by ensuring confidentiality under HIPAA guidelines and agency policies
- Follows infection control policies as needed
- Assists with on-call requirements for certified sites (must have land line in private residence)
- Acts as liaison with care coordinators and outside agencies for community resources and service delivery
- Responsible for administering medications, as needed
- Coordinates meetings, as appropriate
- Participates in corporate compliance and quality assurance activities as required
- Conducts special review investigations if needed
- Responsible for completing all required training in accordance with the Agency policy, and maintaining all required certifications and/or professional license requirements.
- Teach Medication Administration class monthly
- Teach Medication Recertification class monthly
- In-services and provides trainings as needed for staff
- Completes weekly site visits in specifically assigned houses
- Provides clinical supervision for assigned IRA's
- Attends appointments with consumers and house staff as needed
- Visits any consumer within 24 hours of discharge from hospital to ensure correctness of discharge paperwork, any med changes, and writes non-goal note after talking with consumer
- Collaborates with clinical team (speech, PT, OT, MH, and nutrition)
- Reviews the new Medication Administration Records monthly
- Completes medication passes as needed
- Monitors pharmacy reviews for completion in a timely fashion
- Attends nurses trainings and in-services as scheduled
- Participates in three month rotation for med recertification passes for Hilltop, Day Hab, and Seniors
- Ensures LPN's in IRA's are trained with glucometers and injections as needed
- Ensures IRA's have working equipment for AMAP's use, i.e. blood pressure cuff, sharps container, thermometer, etc.
- Completes nursing assessments for potential placements
- Performs health systems reviews on every consumer in individually assigned houses yearly
- Communicates with individual IRAs as needed to address clinical, health, medical concerns for consumers
- Other related duties as assigned
Nonessential Functions:
- Maintains awareness of best practices and new developments in field.
- Provide positive leadership role model.

- Must be able to use medical instruments including but not limited to, Glucometers and oximeters.
Minimum Qualifications:
- Current NYS License as a Registered Nurse
- Two years of specialized training and/or one year of experience with individuals with development disabilities or mental retardation preferred
- Valid NYS drivers license with a safe driving record per agency policy; use of personal vehicle is required
Physical Requirements:
- Must be able to walk up and down stairs, prevent an adult from falling, assisting an adult to his/her feet, etc (adults weighing up to 200 pounds).
- Must be able to use standard office equipment including personal computers, phone, fax, copiers., etc.
Working Conditions:
- Work may require occasional weekend and/or evening work.
- Will be required to travel throughout the work day.
