This is a Full-Time position and allowing you to work alternating in home and virtual setting. Please review the below job description and Apply Now if you are interested.Pay Rate: $55-$60/hr.Schedules: Schedules vary.Monday-Friday 8:00AM-6:00PMSat & Sunday 10:00AM-6:00PM_Hourly wage may depend upon experience, education, geographic location and other factors._**Qualifications**Associated Degree from an accredited nursing programNew York State RN License, required.Work experience with UAS-NY or Oasis Community Assessment tool, required2+ years' experience working in-home setting within the last 15 years, requiredWork experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilitiesComputer literacy: ability to navigate new and emerging technology; ability to perform telehealth assessment; highly developed written and oral communication skills; strong interpersonal skills; ability to work in a stressful and changing environment; and proven ability to work with community groupsKnowledge of long-term care services and programsAbility to travel throughout the assigned region, as assigned, including travel to individuals' homes**Highlights**Full county coverage requiredAppointments based on proximity to your home/self-assign your territory**Responsibilities**The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, to assist the Physician in the completion of a Physicians orderThe incumbent may follow up with individuals who have received an assessment through face-to-face communicationsResponsible for reviewing and making recommendations on consumer assessments for long term care services and supportsAssess cases presented and utilizes clinical competencies to review and validate assessment outcomesWork collaboratively to monitor and address clinical quality and gaps in careProvide clinical and educational assistance to support project initiativesDemonstrate communication skills necessary to interpret age-specific responses**If you would like to be considered for this position, Please Apply Now!**Benefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and 401K plan.
