A home health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their team in Queens as a Nurse Assessor.Responsibilities:The Nurse Assessor (RN) will:
- Meet one on one with the person in the adult home to complete the clinical assessments
- Maintain scheduled assessment appointments
- Provide clinical expertise to determine class members' eligibility
- Advocate and recommend additional clinical resources to ensure that the client remains successfully housed in the community
- Perform other duties, as needed
Qualifications:
- 5+ years of experience in a Mental Health role/setting
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
- Experience completing Psychiatric Assessments
- Knowledge of the Community Mental Health System and various community resources
- Solid understanding of Public Entitlements and Enrollment requirements related to Managed Long-Term Care and Certified Home Health Agency services
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Bachelor's Degree in Nursing
- Has experience with geriatric patient care
