Job Description: Job Details Description Seeking a Registered Nurse for Ohels integrated Behavioral Health Clinic to provide mental health, substance abuse treatment and physical health monitoring. Successful Candidate Will:Be a lead member of the Integrative Behavioral Health team, enrolling and monitoring clients into the Certified Community Behavioral Health Clinic ServicesProvide primary care screening and monitoring of key health indicators and health risks (e.g., BMI, blood pressure, tobacco use).Document client care, and collaborate with other members of the patient care team.Manage and coordinate the health care needs of clients to improve access to care and promote physical and emotional well beingProvide high quality, skilled, whole-health services within a rehabilitative and therapeutic model of careActively engage with clients, developing positive relationships that support the client in meeting their goalsAct as a collaborative team member leading and addressing comprehensive health promotionFacilitate preventative and restorative health and well-being education in individual, group and community settingsAct as a liaison between the client and external care providers for whole health needsProvide information and/or referrals to resources in the community for increased support and stabilizationOffer coaching on medication managementDocument observable behaviors and symptomsProvide 1:1 support and intervention for people served as needed.Participate in meetings and communication with other providers as needed. Current NYS RN license and a preferred 2 years of post-graduate experience. Experience in mental health a plus. Excellent salary and benefits with a flexible schedule. This Full time position is located in Brooklyn. Salary: $80,000- $95,000
