Registered Nurse Care Manager - Stony Brook Family and Preventive Medicine, UFPC

Location: Lake Grove, NY

Schedule: Full Time

Days/Hours: Mon - Fri 8-8; Occ Sat (hours vary)

Salary Range: $99,806 - $110,145 (pay will be determined based on experience, preferred qualifications, and education)

Responsibilities:

SUMMARY: The Department of Family, Population & Preventive Medicine seeks a Registered Nurse to provide high quality individualized direct patient care utilizing the skills and knowledge of the profession. Provides supervision to clinical staff. They will perform appropriate patient testing, and direct patient care to patients within the scope of his/her licensure, and in accordance with patients treatment plan and/or physician orders. They must safely perform all patient care responsibilities in accordance with professional standards and the organizations policies and procedures. Utilizes physical assessment skills to assess the needs of patients. Develop and organize a plan of care in consultation with the treating physician, as indicated within the various subspecialties of the Department and administer medications and treatments according to the specific orders of the ordering physician, and document in the patient record. Follow all HIPAA and OSHA guidelines. Triage patient phone calls, work with care manager and care coordination team. Requires a high-level problem solving and analytical skills with the highest ethical standards. The incumbent must be self-directed and self-motivated and able to multi-task in a dynamic, fast-paced environment with a high level of productivity and accuracy.

Job Duties & Essential Functions:

  • Supervision of clinical staff at all locations, oversee the care coordination team; PCMH working group and work with the institutions PCMH leaders.
  • Oversee effectiveness of once daily huddles in terms of assisting with patient care, disseminating procedural information and important updates.
  • Oversee all Aspects of Vaccine Management (ordering, inventory management, freezer and refrigerator temperature logs, NYSIS Data input, printing records, granting access) & Patient Administration (charting, consent forms, vaccine log)
  • Manage Clinical Staff Personnel Tasks
    • Includes but not limited to:
      • Responsible for staff recruitment/interviewing
      • Oversee New Staff Onboarding
      • Performance evaluations & Performance Plans
      • Performance Improvement Plans
  • Work with Management Team to review quality and customer service metric reports, identify, and participate in QI projects.
  • Assist Providers with immediate on-site care management/care coordination services including: creating care plans for highly complex patients, education for care management patients, assisting patients with social determinants of health (e.g. transportation, unemployment, food, etc.).
  • Responsible to contact patients, facilitate Transitional Care Calls; transition of care (ToC) approximately 80 calls per month. Follow up with patients discharged from the emergency room, facilitate re-education and follow up appointments. Approximately 35 calls per month.
  • Work with providers, to implement PCMH policies, follow insurance company and institutional quality measure guidelines for identifying patients for care management services and gaps in care.
  • Annually review and update PCMH policies and procedures covering all existing current certification areas.
  • Create and implement new PCMH policies and procedures based on annual new additions to certification requirements.
  • Run, review, and summarize PCMH reports related to patient recommendations, timeliness of response to advice and access. Including implementing, reviewing, and summarizing patient surveys quarterly at both locations to submit to PCMH. Run, review, and summarize the after hours appointment availability. Run, review, and summarize the clinical advice after office hours report for PCMH.
  • Prepare PCMH annual reports related to care management eligibility and provider adherence to requirements for care plans.
  • Create and implement training materials for clinical staff to ensure adherence to policies and procedures.
  • Follow up with Care Management Patients including keeping track of a daily spreadsheet of all patients enrolled in care management. As well as alerting nursing staff and providers in both locations of their upcoming care management appointments. Create/update care plans with patients as needed for chronic diabetes patients, multiple hospitalizations, elevated PHQ-9 score, and social determinants of health. Ensuring patients are scheduled for care management appointments.
  • Triage patient calls, contact appropriate care provider as necessary. Includes but not limited to:
      • Patient calls from call Center
      • Portal Messages
      • Providers requesting clarification about patient medical complaints
      • Arrival screener with questions about symptoms
      • Calls from Home Care nurses with questions
      • Labs calling with Critical Values
      • Staff with questions when returning calls
  • Perform patient care duties; perform appropriate patient testing and direct patient care to patients within the scope of his/her licensure, and in accordance with patients treatment plan and/or physician orders; administer medications and treatments according to the specific orders of the ordering physician, and document in the patient record. Train new staff members, as well as re-training existing staff members as needed, on all patient care duties.
  • Provide education to patients as needed by providers including: 15-30 minute diabetes or obesity medicine education. The RN will spend approx. 15-30 minutes with the patients and educate them on how to check their blood sugar levels at home, how to use their continuous glucose monitor (CGM), how to administer a GLP-1 medication, etc.
  • Demonstrate courteous and professional behavior when dealing with patients.
  • Resolve patient inquiries for solution. Manage situations where police, CPS, or EMS need to be called in to handle escalating problems.
  • Perform all other duties as assigned by management.
Qualifications:

Required Qualifications:

  • Associates degree in Nursing. (New York State BSN in 10 Law - Must attain Bachelors in Nursing or higher within 10 years of initial licensure. Current RNs, as well as those currently enrolled or pending acceptance into a program preparing registered nurses effective date of this act (which is Dec. 19, 2017) are grandfathered in.)
  • 3 years of nursing experience.
  • Basic Cardiac Life Support (BCLS) required.
  • Current registration to practice as a Licensed Registered Nurse in New York State.
  • Must have excellent communication skills.

Preferred Qualifications:

  • Bachelors degree in Nursing.
  • Experience with care coordination methods preferably in an academic practice and/or PCMH primary care, Care Management experience.
  • Experience with Transitional Care Management.
  • Advanced Cardiovascular Life Support (ACLS).
  • 1 year inpatient experience; 5 years outpatient experience.

Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate with patients, staff and medical providers. The employee must be able to exchange accurate information in these situations. This position is active and requires the employee to constantly move about in the office to assist patients, staff and medical providers. Any additional physical demands will be outlined and provided by management.
