Summary of Position: Working in conjunction with the scheduling coordinator and all patient services staff under the direction of the RN Office Manager, RN Case Manager will:
- Ensure quality and safe delivery of home health care services
- Implement current nursing practices as defined by New York State Registered Nurse licensure
- Promote effective written and verbal communication with colleagues, clients and health care professionals
- Promote education for home health aides, clients and families
- Supervise and train home care workers
Qualifications:
- Candidate would ideally have experience in a clinical environment, preferably in a community setting
- Can work independently
- Maintain strong organizational and communication skills
Requirements:
- Registered Professional Nurse with a current license to practice nursing from New York State Department of Education.
- At least 1 year of full-time experience as a Registered Nurse in a hospital or residential health care facility is required.
- Home care experience and exhibited instructional abilities desired.
- Valid New York State drivers license
