Join our team and be a part of making a positive impact on the lives of those we serve.Position Overview:We are currently seeking a skilled and experienced Home Health RN Case Manager to join our team in Carson City. As an RN Case Manager, you will play a crucial role in coordinating and providing comprehensive home health services to our patients.This is a Full-Time position with a competitive compensation package ranging from $104,000-$115,000/year, based on a Pay Per Visit Model.Responsibilities:Conduct initial assessments and develop individualized care plans for patients.Coordinate and provide skilled nursing care in a home setting.Collaborate with interdisciplinary healthcare teams to ensure the best possible patient outcomes.Educate patients and their families on healthcare management and promote self-care.Monitor and evaluate patients' progress and adjust care plans as needed.Qualifications:Current and valid Registered Nurse (RN) license in the state of Nevada.Minimum of 1 year of experience in home health or a related field.Strong clinical and assessment skills.Excellent communication and interpersonal skills.Ability to work independently and as part of a collaborative team.Let Better Growth Come To You!#MH-BLAt Mission Healthcare, we believe in fostering an inclusive workplace where diversity is valued and every employee feels respected, accepted, and empowered.
