Weekly Pay Range - $2605 To $2895 Job Title: Registered Nurse Job Specialty: Case Manager Job Duration: 13 Weeks Shift: Day 4x10-Hour (08:00 - 18:30), Friday to Monday Guaranteed Hours: Not specified Experience: Minimum 2 years required License: Valid CA RN Licensure Certifications: BLS (AHA) Must-Have: - EPIC Experience- 1 year experience with knowledge of post-acute levels such as Home Health, Hospice, Palliative Care, Skilled Nursing Facility (SNF), Long-Term Acute Care (LTAC), Board and Care (B&C), Sub-Acute, and Acute rehab. Job Description: - Responsible for Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience.- Complete timely Initial Assessments, updated notes, and final notes.- Conduct referrals to post-acute levels of care.- Coordinate with the multidisciplinary team to ensure discharge is on track.- Possible float to cover sick calls/PTO depending on census requirements.
