Our Community Corners practice utilizes registered nurses in a computer-based role managing triages within our EMR system. Triage content includes acute/chronic symptoms, warfarin management, lab/imaging results, medication refills, referrals, and hospital follow-up coordination. RNs work closely with providers and office staff to facilitate patient care. No on-call, no nights/weekends. The office is closed on major holidays.Roles and Responsibilities:
- Communicate with patients via phone and portal messages
- Assess and triage patient health concerns
- Relay results and guidance to patients from providers
- Reconcile medications after hospital discharge
- Manage INR and warfarin dosing via DoseResponse system
- Occasional nurse visits for injections and patient follow-up
- Assist clinic staff and providers with acute needs onsite
- Build long term relationships with patients
Required Skills and Experience:
- Graduation from accredited RN program
- Current NYS licensure as a registered nurse
Preferred Skills and Experience
- Experience with patient triage, outpatient care, and/or computer-based nursing

- $35.63 to $44.18 per hour
