POSITION SUMMARY The Nurse Navigator will work in a fast paced, multi-line, service offering health information and triage to both the community and other health care professionals through the telephone and internet computer technology. The Navigator will have an opportunity to expand their clinical expertise by working in this team environment on the cutting edge of the nursing discipline. This position is responsible for the triage of health concerns via the telephone and to provide the callers with health advice and information, using established computerized guidelines and documentation standards. The ability to prioritize incoming and outgoing calls based on clinical acuity is a must. The Navigator will be able to assess patient concerns via telephone and provide nursing advice. The Navigator will receive phone calls relating to appointments, referrals, health information, and registrations. The Nurse Navigator Pre Admission Testing will exhibit strong leadership, team building and decision making capabilities. Has the ability to set priorities, identify and resolve problem and delegate effectively and efficiently. This position requires excellent communication and organizational skills for managing workflow, patients, patient families, physicians and other under stressful conditions. The Access Center Triage RN demonstrates reliability while maintaining confidentiality and the ability to work independently and as part of a team. QUALIFICATIONS EDUCATION: Graduate of an accredited School of Nursing required. BSN preferred. LICENSE/CERTIFICATION: Current NJ RN license required; Current Drivers License required. EXPERIENCE: A minimum of 2-5 years of nurse triage in a healthcare/ambulatory setting required. The position requires an ability to determine an appropriate disposition for the situation utilizing excellent assessment skills and sound clinical judgment in an independent environment. Outstanding verbal communication skills and the ability to express self in a clear and compassionate manner is a must. The incumbent must possess the desire to work in an environment that supports self-direction and provides diverse clinical opportunities that utilize a high level of clinical competence. PERFORMANCE EXPECTATIONS Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position. WORK ENVIRONMENT This position requires sitting, standing walking, stooping and crouching a majority of the workday. Ability to lift and transport a minimum of 15 pounds. Works with such equipment as computer terminal, fax machine, printer and copier. REPORTING RELATIONSHIP This position reports to department leadership and may supervise staff as assigned.
