Job Description:
Qualifications: Current license and registration as an R.N. in New York State Responsibilities:
  • Evaluate each individual's needs regarding nursing care on a continuing basis by doing physical assessments and making recommendations based on findings from these assessments.
  • Develop, review, and update nursing care plans on a quarterly basis or semi-annual basis (depending on program type) or more often as needed.
  • Complete monthly reports (in IRA programs)
  • Prepare for and participate in the interdisciplinary team process with attendance at quarterly, semi-annual, and annual review meetings.
  • Complete records for quarterly, semi-annual or annual meetings.
  • Regularly monitor individual's medication administration records to ensure that medications are administered properly.
  • The R.N. will administer any medication that is to be administered by injection.
  • Report medication errors appropriately and give instructions on how the error should be handled to ensure the wellbeing of individuals. Physical evaluation of the individual should be performed.
  • Regularly monitor sleep charts, BM charts, weights, vital signs, menstrual records, seizure charts, and any other record deemed medically necessary.
  • Monitor the effects and watch for adverse effect of medications - allergies, interactions, and contradictions.
  • Review clinic summaries and ensure that all physician orders and recommendations are followed. Speak to physicians either by phone or in person regarding recommendations related to clinic visits.
  • In conjunction with QIDP and the Residence Manager, ensure that all medical recommendations are brought to resolution.
  • Ensure that all medical appointments are scheduled and needed follow up appointments are made. Provide information to Residence Manager regarding needed appointments.
  • Maintain appropriate medical records.
  • Prepare, schedule and provide needed in-service training for staff in any needed area.
  • Certification of AMAP staff upon completion of initial training and annually thereafter
  • Ongoing supervision of AMAP staff.
  • Collaborate with QIDP, Residence Manager, and all interdisciplinary team members regarding individuals' plan of care!
  • Be available on-call to answer questions and respond to medical emergencies.
