Essential Duties And ResponsibilitiesProvides direct care to patients, carrying out written, verbal (in emergencies) and protocol/standing orders by the medical provider that is safe, empowering and satisfying with physical, mental and emotional well-being, supporting optimal health and safety for all patients throughout Health and Wellness sites.Work directly with patients to obtain their medical/surgical history, obtain and monitor vital signs, perform phlebotomy, prepare patients for diagnostic CLIA-waived testing, perform tests, perform wound care, administer medications by mouth, injection, sublingual, and inhalation, and assist medical provider in the examination room.Delivers nursing practice within the HWC is safe, effective, efficient, equitable, timely, and patient-centered.Delivers nursing care according to the standards of care defined within the American Nurses Association (ANA) Standards of Clinical PracticeProvides chronic care disease support to the patients in the form of phone call contacts, nurse visits, and care coordination and management of appointments and medical devices, referrals, and transitions of care needs and education.Performs physical assessments and documents all aspects of care in the electronic health record.Triage walk-in patients and patients calling in telephonically with clarifying questions, physical/mental health complaints or concerns, medication questions, etc. In instance of telephonic triage, directs caller to appropriate level of care, individual/support person to address their concern and maintain direct contact with site nurses and providers to communicate imminent and pending patient needs. Triage nurse may address client's needs/concerns at the point of contact if appropriate.Manage medication refills for providers in accordance with medication refill protocol.Directly participates in case discussions and prepares for and submits patients for discussion/review in weekly multi-disciplinary team meetings and daily huddles.Assists with management of Medication Assisted Treatment client care coordination and service delivery, including administering Vivitrol injections, ordering medications and arranging for delivery and managing the MAT panels for providers when needed. Conducts nurse visits with the medication assisted treatment patients in accordance with nurse protocols.Coordinates care for patient's needs including visiting nurse services, home companion services, prior authorizations, medication management, durable medical equipment, referrals and communicating results to patients as directed by provider, etc.Demonstrates strong nursing knowledge base and critical thinking skills in job performance.Ensures continuity of care is maintained in all aspects of patient care.Maintains flexibility pertaining to cross training throughout all health centers, clinical programs and service lines and coverage at other Health and Wellness sites as it meets the needs of the department.Contributes to medical staff's effectiveness by identifying patient care issues that must be addressed; providing information; recommending options and courses of action; and implementing medical providers' directives.Serves as liaison to medical providers and other staff.Protects patients and employees by adhering to infection-control policies and protocols, not limited to hand hygiene and personal protective equipment usage.Must be able to pass a respirator fit test at time of hire and on an annual basis.Participates with regular staff meetings focused on coordinating care within an interdisciplinary team.Supports training, education and competencies for the clinical support teamPerforms job responsibilities consistent with standards for best practice nursing, including excellent communication with all providers to ensure the delivery of high quality care.Regulatory and Corporate Compliance ResponsibilitiesUnderstands and adheres to Wheeler Clinic's compliance standards as they appear in the clinic's Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy.Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HI-TECH, Infection Control, Sexual Harassment, etc) and specific to the department and position.Keeps abreast of all pertinent federal, state and clinic regulations, laws, and policies as they presently exist and as they change or are modified.Performs other related duties as assigned.Wheeler Clinic is an equal opportunity employer.
