Under the guidance and direction of the Home Care Manager, the Home Care Case Manager RN will participant with planning, organizing, implementing and reviewing of home care services, as well as providing direct physical nursing care to the patient. The Home Care Case Manager RN is responsible for providing the highest quality of care in their PACE participants homes and ensuring safety at all times according to accepted nursing practice standards, which includes health teaching, delegation to other nursing and paraprofessional staff, and acting as a member and a liaison between the PACE CNY Interdisciplinary Team and the participant and family/caregivers.
