Responsibilities:
- Perform initial nursing assessments and ongoing cognition screens at start of care and develop an individualized care plan for each new client including recommendations for care hours.
- Perform follow up visits as per state licensing requirements and at client status changes.
- Accurately monitor and record data regarding patients' conditions; document all provided care services.
- Collaborate with community care partners to support the provision of quality care to each client.
- Supervise, support and training of CNAs to ensure that they are fully equipped to provide the highest quality care to our clients.
You must possess the following qualifications and experience:
- Must be a Registered Nurse licensed in Maryland.
- Must possess sound knowledge of nursing principles, procedures, and standards of practice as determined by state and federal regulations.
- At least 1 year of experience as a RN.
- Experience working with seniors in home care preferred.
- Experience in care coordination between, physician, clients, referral sources
- Excellent communication skills.
- Great problem-solving skills.
- Basic IT Skills.
