Job Summary: We are seeking a compassionate and skilled Registered Nurse to provide home health care to clients in designated geographic territories. The ideal candidate will follow care plans, administer treatments, and coordinate care management while demonstrating empathy and strong communication skills.
Key Responsibilities: - Follow designated care plans tailored to each patient's needs.
- Conduct home visits to clients within assigned geographic areas.
- Perform duties including medication administration, wound care, and various treatments and procedures.
- Monitor clients' conditions and report any changes to the Clinical or Client Services Manager.
- Execute and document doctors' orders accurately.
- Conduct client assessments as required.
- Manage and coordinate care effectively.
- Utilize a state-of-the-art touch pad tablet to document observations, interventions, and evaluations related to client care.
- Demonstrate caring and empathy in all interactions with clients and their families.
- Take and track vital signs; measure and administer medications as prescribed.
- Provide wound care and catheter care as needed.
- Assist with feeding and bathing patients, ensuring their comfort and hygiene.
- Maintain clean bedding and track and order necessary supplies.
- Be willing to work a variety of shifts.
- Use reliable transportation to reach clients' residences.
- Provide nursing care confidently without oversight.
- Build rapport with patients and their family members through excellent communication.
Required Qualifications: - Associate's degree in nursing (required).
- 3+ years of nursing experience in a residential setting.
Preferred Qualifications: - Bachelor’s degree in nursing (preferred).
Education: Bachelors Degree
