Salary: $85,000-$90,000 plus benefits Job Summary: The Home Health RN will be responsible for scheduling and opening cases, conducting initial assessments and ongoing re-assessments of clients' conditions, and managing the orientation and in-service training activities of Home Health Aides. Job Responsibilities:
- Open new cases and implement personalized client care plans.
- Ensure all required signatures are obtained on relevant documentation.
- Coordinate services and consult with clients and their families regarding care.
- Prepare and maintain comprehensive client files.
- Refer clients to additional services as necessary.
- Inform Case Managers and other personnel of any changes in client needs.
- Evaluate the effectiveness and outcomes of care provided.
- Supervise and train Home Health Aides, ensuring they meet all federal, state, and local regulations.
- Perform regular chart audits to ensure documentation accuracy and compliance with regulations.
- Conduct ongoing in-service training activities for Home Health Aides.
Qualifications:
- Graduate of an accredited school of professional nursing with an active Registered Nurse license in Ohio.
- Minimum of two years of experience in home health care.
- At least one year of clinical nursing experience or two years of home health care experience.
- Demonstrated supervisory experience.
- Effective verbal and written communication skills.
- Strong organizational and time management abilities.
- Proficient in using computers and electronic health record (EHR) systems.
- Valid Ohio driver's license and reliable, insured transportation.
