Why does the job exist?A Family Connects NM home visiting nurse provides home visiting to all families with newborns in the determined catchment area. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The home visiting nurse will work in a team environment of clinical and non-clinical staff in order to schedule, visit, and support families in the community using the evidence-based Family Connects NM home visiting protocol. In addition, the home visiting nurse will help caregiver(s)/parent(s) understand that having a newborn is a universal time of need; the variable is the amount of need each family has. The FCNM team continuously works to de-stigmatize home visiting, a notion that is critical to the success of a universal approach in supporting all families with a newborn. How does it get done?1. Follow the Family Connects home-visiting protocol. Keep records of client observations and activities as required. Documentinformation for family assessments and program evaluation. Record information in real time via in home use of computerized record system.2. Follow the Family Connects scheduling protocol. Visit target area hospitals as needed to schedule home visits for new mothers.3. Provide a health and physical assessment of mother and baby in the home to determine well-being.4. Assess family needs in 12 domains of physical and psychosocial well being, including depression, substance abuse, and domestic violence. Exercise clinical skills in appraising situations and making decisions.5. Establish a trusting relationship with families during in-home visits and engage the family to establish a family support plan Who are the customers?All caregivers, newborns and community partners that work to support families. Ideal CandidateThe ideal candidate for this position must possess and maintain a current New Mexico Registered Nursing license or eligible forlicensure in accordance with Nursing practice Act 61-3-2 through 61-3-+31 NMSA 1978 and 16-1-1 through 61-12-20 NMAC, as applicable.referred skills include (a) Spanish language proficiency, (b) clinical experience related to domestic violence, substance abuse and/ or depression, (c) one or more years of experience in child/maternal health, social work, or other related field, and (e) Certified Lactation Educator or Consultant.3. Ability to work in a team with clinical and non-clinical staff.4. Ability to work with nurse supervisor to implement a practice improvement and clinical development plans. Minimum QualificationMust possess a current license to practice as a Registered Nurse in New Mexico OR eligibility for such licensure in accordance with New Mexico requirements OR Eligibility for a temporary license issued for employment at a specific institution in New Mexico and three (3) years of experience as it pertains to the essential duties and responsibilities of the classification. Employment RequirementsMust possess and maintain a valid New Mexico Driver¿s License. Must process and maintain a current Defensive Driving CourseCertificate from the State of New Mexico or must pass and receive Defensive Driving Course as a condition of continued employment. Successful completion of HIPAA (Health Insurance Portability Accountability Act), and successful completion of CLIA (Clinical Laboratory Improvements Act) trainings are required sixty days (60) of employment. Position will require Blood Borne Pathogen training and successful completion of (CPR course within thirty (30) days Working ConditionsServices are provided primarily provided in the home, but can also include clinics, auxiliary and community sites. Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage with extended periods of office noise, bending, standing and lifting. Ability to lift up to 50 lbs. Services are provided in a clinic with diverse racial and ethnic populations, many who do not speak the English language.
