- Provide skilled nursing care to hospice patients within our center-based facility.
- Conduct thorough assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Administer medications, monitor vital signs, and manage symptoms to ensure patient comfort.
- Educate and support patients and their families regarding end-of-life care and symptom management.
- Coordinate care with other healthcare professionals within the center.
- Offer emotional support, counseling, and education, addressing the unique needs and concerns of patients and their families.

About You:
- Active RN license in the state of practice, or eligibility to take NCLEX for new grads.
- Minimum of 1 year of experience in hospice, palliative care, or a related field preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to collaborate effectively within a multidisciplinary team.
- Compassionate and empathetic approach to patient care.
- Knowledge of hospice regulations and practices preferred.
