Job Description:We have an extraordinary opportunity for a Registered Nurse (RN) to join a compassionate, skilled, and dedicated hospice team. - Provide direct care to patients using accepted nursing standards and consistent with facility policies.
- Administer medications and treatments as prescribed, documenting interventions accurately.
- Perform thorough and accurate assessments, providing appropriate follow-up and documentation.
- Conduct hospice admissions and initiate plans of care.
- Collaborate with the Interdisciplinary Team to perform ongoing assessments and care plan updates.
- Administer symptom management interventions based on patient and family needs.
- Manage technical equipment, including feeding pumps, medication pumps, and suction machines.
- Maintain proficiency in intravenous access devices and administer intravenous medications.
- Accurately document in the electronic medical records (EMR) system, ensuring timely completion.
- Coordinate care with clinical and support staff and keep physicians informed of patient progress.
- Attend hospice-sponsored in-service training and participate in Interdisciplinary Team meetings.
Qualifications:
- Valid and current New York State Registered Nurse (RN) license; BSN preferred.
- At least two years of recent clinical nursing experience, with strong skills, including one year of recent Med-Surg experience.
- Commitment to taking and passing the hospice and palliative care nursing certification exam (CHPN) within three years of hire (if not already certified).
- Proficiency in Microsoft Office, EMR systems, and the ability to learn new software programs.
- Hospice admission and end-of-life care experience is preferred.
- Excellent written and verbal communication skills.

- Access to a vehicle, valid driver's license, and auto insurance to travel to patient locations.
- Flexible schedule, including weekend rotations as needed.
