This role offers PPV rates $90-95/point (plus mileage) with an admin rate of $65/hr.Key Responsibilities:Maintain 30 points per week.Conduct comprehensive assessments of patients' health status and develop individualized care plans.Administer medications, treatments, and therapies as prescribed by the physician.Monitor patients' conditions and report any changes or concernsEducate patients and their families on disease management, medication adherence, and self-care techniques.Collaborate with other healthcare professionals, including therapists, social workers, and home health aides, to ensure holistic care delivery.Maintain accurate and timely documentation of patient assessments, interventions, and outcomes in electronic health records.RequirementsCurrent Registered Nurse (RN) license in the state of CaliforniaReliable and insured vehicle and valid driver's license.Minimum of 1 year of clinical nursing experience, preferably in home health or a related setting.Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal skills.Ability to work independently and as part of a multidisciplinary team.Proficiency in electronic health record documentation.BenefitsComprehensive benefits package, including medical, dental, and vision insurance.401k3 weeks of paid time offMileageSurcharge offered if patient is outside your territory
