Job Description:
GENERAL PURPOSE OF JOB Ensures that all medical/health related services are provided within the scope of agency policies and procedures and in compliance with all regulatory agencies. ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Ensure all individuals receive adequate and timely healthcare oversight, monitoring and follow up.
  • Ensure that all physicians' orders are followed and implemented as prescribed.
  • Ensure that all follow up appointments ordered by a physician are completed as prescribed.
  • Ensure that all medications as ordered by a physician are administered as ordered.
  • Ensure that there are no outdated medications in the designated medication storage areas in all programs. Once a month, the RN will check the paperwork to ensure that staff are doing the med counts and checking the expiration dates.
  • Ensure that staff are provided guidance on Individuals' specific protective oversight needs pertaining to healthcare and safety needs.
  • Train staff on DNR/DNI protocols and know where the paperwork is, call Hospice with concerns. If deceased, call Hospice.
  • Participate as a member of the Interdisciplinary Team.
  • Participate in the pre-admission evaluation study and plan.
  • Participate in the evaluation study, program design, and placement of each resident at the time of admission.
  • Develop a written plan for each resident to provide nursing services as a part of the total rehabilitation plan.
  • Annual and PRN are re-evaluated and updated nursing service plans and programming.
  • Make recommendations as needed to appropriate community agencies (hospitals, rehabilitation centers, home health agencies, clinics, laboratories, etc.), individual's Primary Care Physicians will make referrals.
  • Will ensure that trainers are providing, In-service staff training in the following areas: detecting signs of illness requiring medical or nursing intervention; reporting to medical authorities; implementation of appropriate protective and preventive measures, including accident and illness prevention and any other pertinent training upon request.
  • Conduct the Medication Administration Training and oversight of staff.
  • Provide immunizations for all employees and consumers as required by regulation.
  • Serve in a nursing capacity as “on-call” for all programs as necessary and required by the agency.
  • Supervise LPNs and Direct Care Staff in assessing the health status of clients on a routine basis.
  • Follow all OPWDD regulations pertaining to administration of Health Care services.
  • Implement all Plans of Corrective Action (POCAs) pertaining to Nursing services and healthcare provision.
  • Updates the manager to review the medical that is in place.
  • Any other duties/responsibilities as assigned by Immediate Supervisor and/or Executive Director.
EDUCATION AND EXPERIENCE
  • RN degree from an accredited School of Nursing or New York State Licensed Nurse Practitioner.
  • 3 years' experience with the IDD population preferred.
SKILLS Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Math: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals; ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages. Reasoning: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical form and deal with several abstract and concrete variables. CERTIFICATES, LICENSES, REGISTRATIONS
  • New York State Registered Nursing license or Nurse Practitioner License
  • The employee is required to successfully complete the required training.
  • A valid NYS driver license with driving record that meets the insurer's standards is required.
PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee in order to successfully perform the essential functions of this position. While performing the duties of this position, the employee is regularly required to stand and walk. The employee is occasionally required to sit, climb, balance, stoop, kneel, crouch and crawl. The employee must be able to lift up to 50lbs. All staff must pass all NYS Justice Center Background Check Requirements, and Physical. In addition, the employee has a duty to report any suspected violations of the law (including any motor vehicle infractions) to the Human Resources Department.
