As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.ESSENTIAL FUNCTIONS:
- Participates in daily morning meeting.
- Provides medication management and planning/coordination, including injectable medications, to consumers in the community.
- Maintains and updates monthly IM calendar.
- Administer IM's and prepares consumers oral medications for weekly delivery by the clinical team.
- Provides staff education to maintain familiarity with program policies and procedures regarding medication management, and secure record storage.
- Coordinates with Team prescribers and pharmacy to ensure consumers receive medications and refills.
- Transcribe orders and updates Medication Administration Records.
- Logs in medication deliveries and completes toxicology request forms.
- Monitor vital signs and side effects of medications and report findings to the Team
- Completes annual nursing assessments and daily progress notes.
- Assess for physical health needs, and make appropriate referrals to and collaborate with community physicians
- Performs other duties, consistent with the goals and objectives of the program, as directed by the Team Leader.
Salary: $87,000 to $92,000 AnnuallyQualificationsSKILLS AND KNOWLEDGE:
- Experience understanding various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery.
- Familiarity with and ability to use an Electronic Health Record system.
- Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
- Ensure progress notes are completed within 24-hours of the delivery of the service and are available to be read in the daily organizational meeting.
EDUCATION AND EXPERIENCE:
- Licensed Registered Nurse in New York in conjunction with either an Associate's, Bachelor's or Master's degree.
- Experience serving the mentally ill/mentally ill chemically addicted (MICA) population.
