Summary

This position will entail treating geriatric and disabled young adult patients in their home environment based upon the needs specified for the Registered Nurse. All necessary equipment will be provided as well as adequate instructions on each individual patient and their needs. Evaluations and treatments will be regular practices for the Registered Nurse to conduct.

Responsibilities

• Point based system of 7 points daily to be considered full time
• Intakes, recertifications, follow-ups and skilled visits
• Responsible for opening and closing all patient visits
• Documentation Systems/ EMR: OASIS for Medicaid and Medicare and AccessEMR

Requirements

• Graduate of an accredited school of nursing
• Connecticut State RN License in good standing
• BLS Certified.
• 1yr Home Care experience preferred

Salary: $80,000-$100,000/yr based on experience
