Job Summary:

This opening is for a full time Registered Nurse with the SD Department of Health, Office of Child and Family Services (OCFS) in Aberdeen, SD. The RN works in collaboration with a healthcare team that works under the supervision of a Regional Manager. The position provides multiple public health services which may include but not be limited to:

* Pregnancy related services; risk assessments, care coordination, referrals, education, physical/mental health screenings and regular monitoring to promote health pregnancy outcomes

* Psychosocial, physical, nutritional, and other health related assessments within the RN scope of practice for individuals and populations

* Development and implementation of plans of care at the individual, community, and regional level. Establishing measurable outcomes to meet client- or population-centered needs

* Documentation of care and outcomes, maintaining accurate records to facilitate continuity of care. Evaluating interventions

* Breastfeeding education and support

* Linking clients and families to health and social resources

* Immunization services

* Promotion of oral health, application of fluoride varnish

* School based preventive health screenings and education

* Health and safety education for individuals, groups, community

* Developmental and Social/Emotional screening and education for infants and children

* Infant Safe Sleep promotion and education

* Collaboration with community partners and providers to enhance referral pathways and explore opportunities to partner in new and innovative ways.

* Community public health emergency response planning

Work week may include days and hours outside of standard Monday thru Friday 8 a.m. to 5 p.m. Travel to satellite sites including overnight stays may be required for service delivery, trainings, or meetings. Lifting required to do anthropometric measurements of infants and children

Licenses and Certifications:

* Licensed as a Registered Nurse to practice in South Dakota. Basic Life Support certification.

Knowledge of:

* Public health principles and practices

* Maternal, infant, and child health and nutrition

* Child and adult immunization recommendations

* Infant/child developmental milestones

* Scope and standards of nursing practice

Skills/Abilities to:

* Work in multiple computer system software applications

* Work effectively with culturally and socioeconomically diverse populations and with interpreter services

* Multitask, prioritize complex situations, and adapt to quickly shifting priorities

* Be organized, plan ahead, and set goals

* Motivate and lead individuals/families/community in identifying strategies to help achieve and maintain optimal health

* Provide health information effectively via one-to-one or in group settings using a variety of formats; in-person, zoom, teams, telemedicine platform, phone, text, etc.

* Share education at the client's level of understanding.

* Prioritize and facilitate referrals within the context of multiple and complicated client needs

* Apply federal and state regulations, policies and procedures in the functions of the position

* Use sound judgement, analysis, and discretion within the framework of defined policies, procedures, and standards of care

* Identify potential barriers affecting client access to services, work to develop solutions

* Access data to identify unreached populations potentially eligible for services

* Recognize and facilitate opportunities for outreach, marketing, and partnership building

* Develop and maintain effective working relationships at the individual, group, community levels

* Maintain professional knowledge, demonstrate empathy, integrity, and professionalism.
