Job Description:
OHI is switching to four, ten-hour shifts in January, 2025. Candidate must be able to commit to this schedule in 2025. RN will be be expected to travel to all northern sites (Lakewood, Brick & Freehold) OR all southern sites (Little Egg Harbor, Toms River & Stafford).Position Summary Reporting directly to the Chief Medical Officer (CMO) and performs oversight and supervision of all clinical support staff (MA's, DA and LPN's). Plans, coordinates and evaluates services utilizing the direct nursing process, comprehensive nursing care and individual case management. Responsible for continuous quality improvement. Facilitates the on-boarding and training of all new clinical support staff. In collaboration with the Chief Medical Officer (CMO) responsible for assisting in the development and maintenance of the clinical support job manuals, job descriptions and core competencies. Assists all providers in developing their teams and provides support as needed. Works collaboratively with Regional Practice Directors (RPD) at all sites. Responsibilities
  • Oversees all nursing functions at the site including administration of medications, referrals to other providers and community health resources, as well as discharge instructions to patients following examinations.
  • Evaluates PAR levels of stock in all departments and approves all orders for clinical supplies at the site before sending to purchasing.
  • Monitors and assists the patient care team in the case of patient overflow alleviating wait time and enhancing customer service.
  • Responsible for making rounds at site for all departments.
  • Provide RN support in medical emergencies.
  • Anticipates potential safety concerns in the environment of care and utilization of infection control policies and intervenes as necessary.
  • Oversees all nursing functions at the health care site assigned including immediate supervision of clinical support staff to ensure compliance with established standards and procedural requirements.
  • Ensures daily team huddles occur in all departments to support teamwork and proactively identify potential challenges/issues and problem solve. Ensures chart preparation is completed, assigning necessary tasks to the clinical support staff.
  • Assists the Chief Medical Officer (CMO) on matters relating to nursing practices and participates in the development of policies and protocols for patient care and the clinical support staff's job manuals, job descriptions and core competencies.
  • Educates staff on new initiatives, forms, policies and procedures.
  • Provides leadership in the development of standards of care and patient/family education.
  • Guides the professional growth of others through role modeling, coaching and/or mentoring of clinical support staff.
  • Provides leadership at the site for initiatives designed to improve the quality of care and clinical outcomes.
  • Responsible for knowledge of and compliance with regulatory and credentialing agencies
  • Reviews and updates clinical policies and procedures regularly to assure consistency with the law and practice standards. This includes establishing, implementing and signing off on clinical competencies for the clinical staff (LPN's, MA's, DA) in alignment with DOH ambulatory care regulations.
  • Using the Scorecard process will complete ongoing performance evaluations of NJIIS coordinator, Dental Assistant, LPNs, MA's.
  • Engages in audits conducted by regulatory agencies.
  • Responsible for Infection Control rounds. Ensures all required logs, documentation and records are precise, up to date and accurately reflect the standards for Infection Control.
  • Responsible for collaborative quality improvement activities and develops Plan-Do-Study-Acts (PDSA).
  • Ensures data collection for quality assurance purposes. Assists in interpretation of data and adherence to guidelines based on results.
  • Ensures the clinical staff maintains the Provider's organizer on a daily basis and that labs are reconciled daily within normal limits. Reports critical labs and abnormal results to Provider following OHI policy.
  • Reports required information to the Health Department for further screenings and follow-up.
  • Receive all critical or abnormal results from laboratory and prompt referral to Providers for management
  • Performs assessment related to triaged calls from the Call Center and walk-in patients at the site and clinical disposition based on level of acuity.
  • Collaborates with Provider and health care team in the assessment, planning and evaluation of patient's care.
  • Proper documentation on EHS using Triage templates and all pertinent clinical documentation related to patient care.
  • Works collaboratively with the PD and CMO to resolve patient complaints.
  • Maintains up-to-date knowledge of EMR utilization and stays informed about EMR clinical update releases.
  • Facilitates clinical onboarding, training, and competency sign-off for new staff.
  • In collaboration with CMO oversees adherence to healthcare regulations and FQHC guidelines, ensuring compliance with local, state, and federal standards.
  • Constantly strives to achieve excellence in day-to-day operations of respective department.
  • Adheres to Corporate Compliance policies.
  • Works collaboratively with all members of the Senior Management and Management Teams to function as a cohesive and professional unit.
  • Participates in all safety programs which may include assignment to Environment of Care (EOC) or Emergency Operations Plan (EOP).
  • Attends all mandatory in-services and meetings required by administration and completes all mandatory in-services in a timely fashion.
  • Is highly accountable for one's own productivity. In order to maintain transparency, all projects must be represented clearly on a project board in Monday.com. Responsible for ensuring the board is always updated and accurate and that information is accessible to those that need it.
  • Other duties as assigned.
Education/Experience/Licensure
  • Associates Degree in related field required.
  • Bachelors of Science (RN's) preferred or other related bachelor's degree is required.
  • Three to five years nursing experience in Internal medicine with a medical center or community health care setting is required.
  • Active State of New Jersey RN license is required.
  • Basic Life Support for providers through the American Heart Association is required.
