Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a CBA ) between Housing Works and the RWDSU Compensation Range: $90,000 - $93,000 yearly commensurate with experience.Overview: The Staff Nurse (RN) is responsible for the delivery of nursing care to clients through assessing, planning, providing and documenting nursing care for a designated group of patients in accordance with the physician's plan of care and Housing Works policies and procedures.Responsibilities:
- Coordinates all necessary client services with other interdisciplinary team members in the comprehensive care planning process
- Performs specialized care in accordance with the competencies and demonstrate initiative in learning new skills and developing competency.
- Maintains accurate documentation of all nursing activities including medication profile, report any unusual changes/occurrences to the attention of the physician.
- Completes clear, concise and accurate clinical notes which documents skilled services and other required documentation appropriately. Utilizes only universal abbreviation.
- Communicates necessary information to the Nurse Care Coordinator.
- Demonstrates professionalism by maintaining high standards of clinical care and staying current on clinical and medical issues.
- Attends and participates in patient case conferences to demonstrate effective coordination and continuity of patient care.
- Work with patient and health care team members effectively to coordinate services and to promote quality care and possible patient care outcomes.
- Cooperates and is reliable with providing on-call coverage.
- Establishes and maintains working relationships and open lines of communication with personnel from physician's office, area hospitals, clinics, etc. to assure flow of information and continuity of care.
- Coordinates patient's medical care with external primary health care providers.
- Responsible for triaging patient care.
- Assess the physical factors relating to the patient's illness, need for care, and response to treatment and adjustment to plan of care.
- Address problems that may have an adverse effect on the patient's ability to respond to treatment and ability to attain optimal functioning. These problems may include, but are not limited to dangers of polypharmacy, review of Physicians instructions, and management of personal hygiene and methods of optimization of ADL skills.
- Administers medication and renders treatment as prescribed by Physician or Nurse Practitioner.
- Completes nutritional assessments and provides instructions on diet/nutrition and seeks consultation as necessary.
- Uses appropriate infection control procedures and safety measure.
- Administrators CPR provides emergency care necessary and maintains current CPR certifications.
- Assists in orienting/precepting new staff when necessary.
- Provides and documents services in accordance with State, Federal, Medicare, Medicaid regulations as applicable.
- Performs other duties as assigned by the Nurse Care Coordinator, Associate Director of Nursing or Director of Nursing.
Minimum Requirements
- Must be a Registered Nurse, licensed by the State of New York.
- Bachelor's degree from an accredited college or university.
- Experience with persons with HIV/AIDS, histories of substance use and/or mental illness a plus.
- Experience working in a setting that promotes interdisciplinary care planning.
- Bachelor's degree preferred.
- Bilingual preferred (Spanish/ English)
