Job Description
A health services network in New York is actively seeking a compassionate, licensed Registered Nurse to provide nursing services to their patients at their hospice / palliative care residence in Dutchess County. In this role, the Registered Nurse will be responsible for providing nursing services to Hospice patients and their families and to ensure that those services are consistent with the philosophy, goals, objectives and policies of the Agency.Responsibilities:The Registered Nurse (RN) will:
- Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team
- Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care
- Document and enter into EMR all assessments and interventions before the end of the shift
- Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings
- Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur
Qualifications:
- Graduate of an accredited Nursing school
- NYS Registered Nurse (RN) license
- BLS certification
Desired Skills:
- 2+ years of Nursing experience
- 1+ year of Hospice experience
- 1+ year of Medical-Surgical experience
