This position provides professional nursing services in the care and treatment of patients within a community health care center and at satellite service provision alias. The incumbent works under the supervision of the Program Administrator, the Assistant Community Health Nursing Director, and in conjunction with medical clinicians. A current Florida nursing license is required. The duties and responsibilities include, but are not limited to, the following:
- Organizes and provides services for patients with exemplary customer service.
- Accurately records all nursing care provided at the time of service.
- Maintains monthly logs related to sterilization method and encounters where education is provided for birth control
and/or sterilization method of choice.
- Develop and implement a case management process for patients that desire voluntary sterilization.
- Works in collaboration with internal and external partners.
- Ensures adherence to nursing protocols, as well as DOH policies and procedures.
- Ensures adherence to billing policies and procedures.
- Accurately applies nursing principles and techniques in the direct care and treatment of patients.
- Makes appropriate nursing assessments of and documents patients health status.
- Assesses, treats, evaluates, and provides health education and follow-up with appropriate cultural competence.
- Assist medical clinician in clinic.
- Interprets and implements provider orders and recommendations to patients appropriately.
- Completes and follows up on referrals as appropriate and accurately documents all clinically relevant information.
- Provides exemplary customer service during telephone and walk-in triage as necessary in consultation with the medical
clinician.
- Properly obtains vital signs and medical history; performs simple laboratory procedures (finger stick, heel-stick, phlebotomy).
- Prepares examination/treatment rooms and equipment.
- Promotes cooperation within the agency and externally by maintaining positive relationships with management team, medical clinicians, nurses, health support technicians, human service counselors, senior clerks, and other staff.
- Develops and maintains an interdisciplinary team approach to patient care and case management.
- May participate in quality improvement activities to include peer review process.
- Receive and review referrals for voluntary sterilization from DOH clinics in Duval; the community partners; and neighboring counties: Clay, St. Johns, Nassau, and Baker.
- Provide family planning and voluntary sterilization counseling to referred eligible patients at UF Health Shands and Urology Consultants.
- Provides appropriate post procedural follow-up to patients and ensures communication with clinicians.
- Assists clinics with OB Triage Process.
- Daily visits to postpartum clients at UF Health Shands and schedule postpartum and/or birth control clinic appointments.
- Provides nursing care to clients at Center for Women and Children, South Jacksonville Family Health and Comprehensive Care Center one day a week, rotating clinics each week.
- Attends clinical staff meetings, in-services, seminars, and trainings for self-improvement, increased competency, and retention of skills.
- Participates in the orientation and staff development programs for persons assisting with client care.
- May oversee the work activities of ancillary personnel.
- Ensures timely completion of all mandatory trainings.
- Ensures accurate and timely submission of employee leave and attendance records, Employee Activity Records (EARS), and travel vouchers.
- Performs all other duties as assigned.
Provides services as required during disasters and performs other related duties as required. Travel is performed in relation to above duties. Access to a personal vehicle and a current driver's license are essential to the performance of the duties described for this position.
Required Knowledge, Skills, and Abilities: - Knowledge of nursing and public health principles, practices, and techniques.

Qualifications: Minimum:
- At least two years' experience providing family planning services in a clinical setting.
- Licensed as a Registered Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27.
- Ability to give injections and immunizations.
- Ability to perform simple laboratory duties.
Preferred: - Family Planning sterilization counseling experience.
