JOB DEFINITION:Work with the Central Nassau Forensic ACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.EDUCATIONAL REQUIREMENTS:Must be currently licensed as a Registered Nurse by the New York State Department of Education and possess either an Associate's Degree, Bachelor's Degree and/or Master's Degree in Nursing from an accredited program*. Bachelor's or Master's Degree preferred.Additional Requirements:

EXPERIENCE REQUIRED:One (1) year of community mental health experience and/or at least two (2) years of mental health experience specifically working in a mental health setting. DUTIES AND RESPONSIBILITIES:
Responsible for the coordination of client services to include scheduling of outside clinic appointments, lab appointments, obtaining random urine analysis (UA) and intramuscular (IM) administration.Ensures the completion of all medical assessments for each client, including coordinating additional services with the psychiatrist.Collaborates with the psychiatrist in the coordination of care for clients to include clinical and educational training of the team as needed; development, maintenance and supervision of all medication administration efforts; supervision of all psychiatric and medical treatment and/or medical interventions.Monitors the pharmacological effects of prescribed psychotropic medications and notifies the psychiatrist of their reported effectiveness.Working in conjunction with the psychiatrist and Program Director, is responsible for the coordination of patient education efforts to include the benefits, risks and possible side effects of prescribed medication. This education effort is not limited to home visits but may also include the coordination of various groups to include but not limited to, medication, wellness, etc.Monitors the client's medication regimen to ensure completeness and accuracy in the provision of care.Assists the psychiatrist by scheduling clients for visits based upon client needs, s/p hospitalization and/or incarceration. The RN will also advise the psychiatrist of all medication renewal needs.Responsible for the direct supervision of the Licensed Practical Nurse (LPN) if applicable, to include weekly client schedules, monitoring of client medication delivery, monthly intramuscular (IM) administration, obtaining blood sugar (BS) and/or blood pressure (BP) when required and/or other tasks as assigned.Must be available (along with other members of the Forensic ACT Team) on an as needed basis twenty-four (24) hours a day, seven (7) days a week, including responsibility for periodic pager coverage.Responsible for making a minimum of four (4) to six (6) client visits daily. These visits are to be chosen at the beginning of each work day and identified on the client work board in the bullpen by the RN's initials.Responsible for entering the documentation for each client visit in the IMA System as instructed by the team leader within twenty-four (24) hours after the client visit. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progressComplete QI Training during onboarding and participate in QI refresher training as needed.Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
