You will utilize your advanced nursing knowledge and skills to manage complex clinical situations independently, while being supported by the PACE Clinical Services Supervisor. Your work will involve thoughtful assessment, planning, implementation, management, and evaluation of patient care, contributing significantly to the patient's healthcare experience.Responsibilities:
  • Assess Patient Needs: Collect and document patient data, prioritize needs, and identify issues requiring referral.
  • Develop Care Plans: Collaborate with patients and families to create comprehensive care plans, integrating interdisciplinary input.
  • Manage and Implement Care: Deliver high-quality care, perform treatments, provide education, and coordinate with healthcare personnel.
  • Evaluate Care: Monitor and document patient care, addressing any variations or issues.
  • Lead and Delegate: Supervise and delegate tasks to ensure effective patient care; contribute to quality improvement initiatives.
  • Collaborate Interdisciplinarily: Actively engage in team meetings and care planning with the Interdisciplinary Team.
  • Contribute to Projects: Participate in additional tasks and projects as needed, demonstrating flexibility and adaptability.
Requirements:
  • Graduation from an accredited nursing school.
  • Current RN license in good standing within the relevant state.
  • Minimum of three years recent clinical RN experience.
  • Familiarity with digital documentation systems preferred.
  • Experience with frail or elderly populations preferred; training available if needed.
  • Reliable transportation, valid driver's license, and auto insurance required.
  • Medical clearance for communicable diseases and up-to-date immunizations.
  • Successful completion of a comprehensive background check (federal and state).
Skills and Abilities:
  • Strong teamwork and communication skills.
  • Proficient in geriatric patient care, understanding their specific needs.
  • Competent in basic computer applications.
  • Effective within RN practice scope, meeting competency standards.
