CNA candidates with less than 1-year hospital experience need manager approval. License: RN, LPN, or CNA Certifications: BLS Must-Have: Experience in caring for geriatric patients (ideal candidates will have SNF or LTAC experience) Job Description: - Care for a variety of patients who are discharge-ready and awaiting community placement.- Manage heavy behavioral and psychiatric cases alongside LTC/SNF.- Perform under a nursing home model with focus on fall prevention and skin integrity.- Adopt a team nursing approach with patient ratios up to 1:9 for RN, and 1:8 for CNA.- Provide less than acute level care; vital signs once a day, full assessments weekly.- Conduct medication administration, dressing changes, enteral feedings, and IV antibiotic administration.- Implement and supervise daily nursing tasks including med passes, dressing changes, and straight catheterizations.- Maintain comprehensive daily nursing notes per shift, documenting patient activities, behavioral notes, and overall impressions.- Coordinate with support staff including CNA, charge nurse, activities coordinator, social worker, and case manager.- Utilize Cerner for charting and work with Alaris and Pyxis systems.- Floating possibilities: RN may float to other areas within their scope, LPN and CNA floating is rare.- Participate in a weekend rotation every other weekend.- Required to follow scrub color guidelines and adhere to safety regulations regarding nails and footwear.- Adhere to scheduling policies, including occasional holiday work as needed and approval process for time off.
