Your Role: s an RN at Alvita Care, you'll: - Conduct thorough assessments and develop personalized care plans. - Collaborate with a multidisciplinary team to ensure top-notch care. - Oversee and support the care provided by LPNs and paraprofessionals. - Manage patient discharges safely and in compliance with regulatory requirements. - Provide guidance and support to patients and their families. Qualifications: - NYS RN licensure and BLS certification. - At least one year of RN experience. - Strong organizational skills and the ability to multi-task. - Experience working with diverse populations. - Valid driver's license or NYS Non-Driver ID card (drivers preferred).
