Population Health RN - Full time CRHC is looking for a full time (40 hours/week) Population Health RN to join our health care team. This position works Monday through Friday, 8am-5:00pm. PRIMARY FUNCTION: Responsible for Advancing Exceptional Care by communicating, implementing, and executing population-level initiatives to improve quality, efficiency, and cost of healthcare services. Accountable for assistance in coordinating, facilitating, monitoring, and evaluating clinically complex patient populations to ensure quality of care and cost-effective use of resources in a manner that is consistent with Clarinda Regional Health Center's (CRHC) mission. QUALIFICATIONS: Education and/or Experience Bachelor's degree or Associate degree with comparable work experience and a commitment to complete within negotiated timeframe. Ability to work with physicians, mid-levels and front-line staff in a collaborative manner to assist patients with exceptional care. Minimum one (1) year experience as a nurse in rural health preferred Certificates, Licensure, Registrations Basic Life Support (BLS) Registered Nurse - Iowa license or compact state
