Job Description:The Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of patients. The Clinic RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice and coordination of care with other health team members in an outpatient clinic setting. Clinic RNs are pivotal members of the care delivery team headed by the physician and have specialized knowledge, skills and ability pertinent to ambulatory care patient needs. Scope of performance includes prioritization and triage of care delivery, implementing established standing orders and protocols, performing procedures and therapies consistent with training and licensure.Job Description: The Registered Nurse (RN) at Northfield Family Medicine and Green Valley Ranch is a dedicated caregiver responsible for assessing, planning, implementing, and evaluating patient care. This RN utilizes the nursing process alongside technology, therapeutic interventions, evidence-based practices, and effective coordination with the healthcare team in an outpatient clinic setting.Clinic RN position is shared between 2 Primary Care Clinics: Northfield Family Medicine and Green Valley Ranch (clinics located within 10 minutes of travel between each other).Position Overview:Join our team at Northfield Family Medicine and Green Valley Ranch and play a vital role in delivering exceptional care to patients with chronic conditions such as hypertension and diabetes. This position offers an excellent opportunity to enhance your clinical skills while collaborating with a dedicated group of healthcare professionals.About the RN Role:As a Registered Nurse in our clinics, you will be responsible for triaging calls, managing MyChart messages, and assisting walk-in patients. Your role will include overseeing hypertension and diabetes management programs, providing patient education on medication management, and acting as a clinical resource for medical assistants. This position requires a commitment to high-quality, patient-centered care.Posting Specifics:

Key Responsibilities:
  • Prioritize patient education by providing comprehensive training on medication management, including self-injection techniques and medication reconciliation. This approach supports the effective management of hypertension and diabetes programs, ultimately ensuring optimal patient outcomes.
  • Triage calls, MyChart messages, and walk-in patients effectively.
  • Perform lab result outreach and monitor medication safety.
  • Serve as a clinical resource for medical assistants and other staff.

  • Current RN License in the state of Colorado or compact license with the ability to obtain Colorado RN license.
  • Associate Degree in Nursing (ASN/ADN) from an accredited program, with the ability to obtain a BSN within four years unless possessing 15 years of continuous RN experience.
  • Current BLS certification endorsed by the American Heart Association.
Preferred Qualifications:
  • Bachelor's Degree in Nursing (BSN) from an accredited institution.
  • One year of experience in a clinic setting or inpatient acute hospital setting with exposure to all age groups.
