Job Type Full-timeDescriptionPOSITION PURPOSE: The Nursing care of each resident shall be the responsibility of a RN Resident Care Manager. He/She will coordinate the nursing functions and tasks for an assigned resident load with physicians and all health care providers. The RCM works in consultation with and under the direction of the Director of Nursing Services, and delivers direct resident care and assists in setting standards and supervising Nursing Staff. The RCM evaluates and assesses resident's physical, emotional and mental status accurately and formulates the Plan of Care, including the resident and/or their responsible party to degree possible. The RCM is accountable for Quality Assurance on a daily basis.ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Functions within the scope of practice for Registered Nurse in the State of Oregon.
The RN Care Manager shall be responsible and accountable for managing the nursing care of residents. The RCM shall ensure maximum independence and self-direction for residents. This includes, but is not limited to:
  • Delivers direct resident care, as needed, acting as a role model and resource person for the Staff.
  • Delegates, trains, evaluates and supports licensed nursing personnel, as needed. Serves as clinical supervisor for all charge nurses and CNAs. Mentors staff to become part of the new team.
  • Coordinates the nursing functions and tasks with physicians and other health care providers and ensuring resident care plans are developed in accordance with resident needs and preferences and documented in accordance with accepted standards of nurse practice and regulations.
  • Documents the admission status of the resident within 8 hours, including, but not limited to skin condition, nutritional status, hydration status, mental status, vital signs, mobility and ability to perform ADL's.
  • Completes the assessment and documents the resident's need for Nursing Services within 14 days of admission.
  • Coordinates the residents Plan of Care with other members of the health care team along with resident and family preferences; completes the written Plan of Care within 21 days of admission and as condition changes - but no less often than quarterly; Communicates Care Plan goals to all members of the team and Nursing Staff and observes that continuity and consistency is being maintained in achieving the goal. Monitors care given routinely.
  • Communicates with resident and family regarding resident care changes and treatment regimes, documenting notification of condition changes to physician and family in the Medical Records; Communicates with licensed personnel on all shifts regarding changes in resident condition or new procedures being implemented for continuity of care.
  • Completes and processes MDS and associated paperwork, including the implementation of CAA's and Triggers, within the federally mandated time frames.
  • Completes medication and treatment RECAPS for assigned residents to ensure correct medication transcription and telephone orders.
  • Follows up with Primary Care Physician of resident for needs related to PRN orders and medication changes.
  • Follows up with Pharmacist recommendations for assigned residents with Primary Care Physician, ensuring a response is received and in compliance with OARS.
  • Monitors, tracks, evaluates and reports infections for the unit. Acts as a resource for nursing staff.
  • Assumes 24-hour responsibility and accountability for resident care on assigned unit. Ensures complete and prompt reporting of incidents with follow-up the Director of Nursing and Administrator.
  • Oversees resident care to promote the highest level of physical, mental and psychosocial functioning possible for assigned unit.
  • Ensures that significant changes in resident condition are communicated to the physician, family or responsible party.
  • Makes daily rounds on unit to ensure resident care needs and environmental standards are met; this includes monitoring of dining room during mealtimes. Ensure all aspects of the 24-hour report books are up to date.
  • Works collaboratively with other members of the interdisciplinary care team to provide holistic care.
  • Reviews clinical records for completeness and accuracy as necessary.
  • Creates and upholds an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment.
  • Works collaboratively with Director of Nursing and Staff Development Coordinator to identify and provide orientation and continuing education for unit staff members.
  • Actively participates in Nursing Supervisory Meetings, Morning Stand-up, Resident Care Conferences, BIMP, Utilization Review meetings, Quality Assurance reporting, and other meetings as requested. Coordinates and assists physicians on rounds and during H&Ps.
  • Participates in the development and implementation of new policies and procedures based on identified needs.
  • Shares 24 hour on-call responsibilities with the DNS.
  • Other duties as assigned by DNS.
NON ESSENTIAL JOB FUNCTIONS include the following:Attends weekly and monthly staff meetings.UNIVERSAL PRECAUTIONS:Exposure to Blood/Body fluids is possible.RequirementsQUALIFICATION REQUIREMENTS:Graduate of an accredited program of nursing education. Current, unencumbered active license to practice as an RN in the state of Oregon. At least 1 year of experience as a supervisor in a healthcare setting. Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities. Knowledgeable of rehabilitative and restorative nursing practices. Proven ability to communicate effectively to staff and residents. Demonstrate leadership abilities.Ability to attain 3 credit hours from an accredited college or university, or 15 continuing education hours, pertinent to management or supervision within 9 months of hire.
