Key Responsibilities
- Administer and monitor medications with accuracy and safety for skilled nursing
- Perform vital sign checks and health assessments to ensure effective health
- Develop and implement personalized care plans with the healthcare
- Provide patient-centered care, assisting with daily activities and fostering
- Educate residents and families on health management, wellness, and
- Document care and maintain accurate patient
- Apply expertise in medication administration and personalized care
- Reassess and adapt personalized care plans to evolving resident
- Conduct routine vital sign checks and update health monitoring
Required Qualifications
- Active LPN or RN license in
- Minimum 1 year of experience in long-term care or skilled nursing
- Expertise in medication administration, vital signs monitoring, and health
- Strong communication and teamwork
- Ability to lift 50 lbs and work evening shifts (2:30 PM - 10:45 PM).
- Experience as an LPN or RN in skilled nursing
