JOB DESCRIPTION:
An RN Admission nurse oversees and assures completion of a comprehensive nursing assessment of a resident's physical, emotional, and mental status upon admissions/readmission Completes all admission paperwork, including MDO, MAR, TAR, Admission checklist, pain, fall, skin, and elopement risk assessments. interventions.
RESPONSIBILITIES:
* Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
* Prepares and administers, and records prescribed medications
* Records patients' medical information and vital signs
* Assesses the patient's ability to do routine daily tasks
* Recognizes and manages geriatric syndromes common to older adults, including cardiovascular, respiratory, gastrointestinal, urinary, hematological, musculoskeletal, endocrine, neurological, psychiatric/psychosocial, integumentary, sensory, and pain problems
* Prepares equipment and aids physician during examination and treatment of the patient
* Communicate with family members regarding patient
* Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format, and on the appropriate forms
* Performs other position-related duties as assigned, depending on the assignment setting
REQUIREMENTS:
* New York State Registered Professional Nurse licensure
* Experience in long-term care facility preferred
* Able to interpret patient information and make critical decisions about necessary actions
* Ability to supervise and work well with and show respect toward other healthcare personnel (i.e., CNAs, LPNs, physicians, etc.), residents, and their family members
* Strong written and verbal communication skills; able to follow written and verbal instructions
* Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
* Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
