Job SummaryNovant Health is hiring aHome Health Case Managerregistered nurse for the standard shift for our New Hanover Regional Medical Center location in Wilmington, NC.We're offering up to a $15,000 sign-on bonus as well as $7,500 relocation. Company car and gas card provided.We 're seeking a registered nurse to join our team in our home health department. We are a very supportive team and always available to help each other out. In this position, you will be responsible for coordinating home care services which includes establishing and maintaining the patient's plan of care. This includes the quality, customer service and financial management of your assigned caseload.
- Schedule:Full-Time, 1.0 FTE, 80 Hours per 2 weeks, Day Shift, Standard Hours (8:00 AM - 5 PM)
- Department:Home Care - Skilled Nursing Unit
- Responsibilites:
- Conducts admissions, recerts, ROC, and discharges for the assigned case load.
- Conducts routine nursing visits as needed.
- Assesses patient needs.
- Coordinates patient care between the patient, caregiver, physician, and other appropriate disciplines.

If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.QualificationsEducation:
- Graduate of an accredited school of nursing required.
- Minimum Associate's Degree required. Bachelor's degree in nursing, preferred.
Experience:
- Minimum 1 year of home health or related RN experience required.
License:
- Current valid North Carolina nursing license required.
Certifications:
- Basic Life Support (BLS) through American Red Cross or American Heart Association required.
