$10,000 / Two (2) Year Agreement# SUMMARY: This position is filled by a registered nurse who, as a result of educational, clinical and leadership experience in medical and pediatric nursing, possesses the knowledge and clinical skills necessary to provide nursing care; applies knowledge to act as a change agent, role model and leader; provides and coordinates the care of patients within the hospice patient population; ensures quality focused and cost-effective care delivery outcomes; utilizes theory, research, and specialized knowledge to plan care for the# hospice patient; functions within a collaborative practice to enhance quality of care; interprets, evaluates and communicates research findings; and participates in continuous quality improvement activities. Able to demonstrate leadership skills in decision making and problem solving. Responsible and accountable for direct patient care for assigned patients in hospice and providing leadership within the nursing team. Responsible and accountable for the care given to the patient and for the decision regarding which nursing tasks can be safely delegated to licensed and unlicensed persons involved in patient care. Must be competent in a wide variety of nursing skills. Responsible for adhering to all standards of nursing care and promoting a team effort among co-workers. Responsible and accountable to know, own and energize the vision, mission, values and standards of Hospice of the Ozarks. Uses professional judgment and critical thinking to prepare, prioritize and execute the patient#s plan of care consistent with patient assessment data and physician#s treatment plans and professional standards of evidence based practice/care. Incorporates the patient#s physiological, psychological, cultural, developmental and spiritual needs into the plan of care. Consistently follows patient care and facility guidelines and procedures in performance of one#s duties. Adaptability is essential for the case manager for Hospice of the Ozarks, as work schedule and daily work load change frequently to meet patient needs. JOB REQUIREMENTS Education: Graduate of an accredited school of nursing.# ADN required, BSN or MSN preferred. Licensure: Current Arkansas or Compact State RN license in good standing.# Valid and clean Driver#s License, reliable transportation and agency required liability insurance to complete home visits.# Active personal automobile liability insurance coverage consisting of $100,000 per person and $300,000 per accident for bodily injury required.# Experience: Minimum of one year acute care nursing experience preferred.# Previous nursing experience in a medical-surgical environment preferred.# Certifications: BLS required Other:# Ability to read, write and communicate effectively.# Basic computer knowledge required.# Driving required. # Safety Sensitive Designation:# This position is deemed safety sensitive in accordance with Arkansas Act 593 governing Medical Marijuana. Occasionally requires lifting of 50 pounds or more of equipment and lifts patients with assist 125 pounds or more with proper body mechanics.# Able to push in excess of 150 pound wheelchair with patient and equipment using proper body mechanics.# Spends eight hours or more in front of computer, monitor or similar screen utilizing keyboard and/or mouse. Must be able to respond quickly in emergency situations of CPR and obtaining equipment. #Must be able to handle critical and highly stressful situations with efficiency and composure.# Has a keen sense of hearing and visual acuity to respond to critical changing patient situations from actual observations.# May be subject to inside environmental conditions, hazards (including electrical currents and exposure to high heat and chemicals) and infectious diseases. Work Environment: Patient Home Setting, Long Term Care Facilities, Assisted Living Facilities, Adult Residential Homes, and Hospice House # Position Type and Expected Hours of Work This is a position outside of the hospital setting, which is open 24 hours a day, 365 days per year. Holidays and work during inclement weather will be required. Work hours will occasional exceed the normal work hours of 8am to 5pm.
