We are your only locally owned, operated and governed home health and hospice agency and are honored to provide services to our neighbors in our communities.In summary the case manager is responsible for the overall care coordination of each home health or hospice patient cared for by the agency from the time of referral to time of discharge. Ensures the timely delivery of appropriate service which promotes quality and cost effective health care outcomes working within established agency policies and home health and hospice regulations. Works closely with visiting nursing staff providing direct patient care to ensure that care is goal directed and patient needs are being met. The case manager acts as a patient advocate and primary contact for patients, caregivers, family, insurance companies, doctors and staff to ensure effective communication regarding patient care. Responsibilities
- Visit patients in their homes for one-on-one visits
- Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
- Directs the coordination of patient care to ensure the educational and nursing goals for patient are achieved
- Schedules and coordinates patient care visits required by Registered and Licensed Practical Nurse staff
- Reviews the appropriateness of patient care, progress toward goals and makes suggestions for changes in the patient care plan as needed
- Gives and receives reports on patients daily to ensure coordination of care
- Case Managers develop, implement and coordinate the discharge plan for patients. Collaborates with other members of the health care team, the patient, caregiver and/or family, to develop and implement and carry out the discharge plan
- Accepts on call assignments as requested
- Identifies and completes the appropriate OASIS data and is knowledgeable of time requirements for data collection at Start of Care, Follow-up, Transfer, Discharge and Death at home and meets time requirements as specified
- Assess patients' conditions and chart observations, and perform evaluation tasks including vital signs and medication review
- Administer medication as prescribed by the patient's physician
- Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient's quality of life
- Provide effective communication to patients/clients, their family members, team members, and other health care professionals
Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing, or vocational nursing
- Current state license as an RN
- Proof of current CPR certification
- Compassionate, friendly demeanor
- Excellent communication skills
- Current driver's license and reliable transportation
- Ability to work a flexible schedule and travel locally
